This World Hepatitis Day on July 28, we want as many people as possible in Ontario to have access to Hep C screening and treatment.
Our integrated care model embeds Hepatitis C care directly into our network of 73 clinics across Ontario. With the ability to connect with patients both in the clinic and at home, we’ve helped thousands of individuals get tested and cured.
Integrated care
By providing free screening and treatment for Hep C, we can help people know their Hep C status and easily access expert services. Each patient is individually managed by a dedicated team, who empower patient success with initial screening, educational resources, check-ins, financial assistance and more.
Initial Screening
We can offer clinic-based screening in person, or at home testing options both with a quick and painless finger prick to determine if there’s an infection.
Education and Support
We ensure our patients have information resources, answers to any questions and link them directly with Infectious Disease Specialists to provide expert Hep C care.
Assistance with Financial Aid to Cover the Cost of Medications
As part of our individual case management, we discuss coverage options with patients and help connect them to financial support as needed.
Ongoing support and Follow-ups
Case managers support patients in maintaining their appointments and next treatment steps.
Access to Medication
With integrated pharmacy services embedded in our care model, medications are conveniently accessed with expert advice.
The need for increased screening
People who inject drugs are much more likely to develop a Hepatitis C infection, with 3 out of 4 new cases appearing in people who share drug injection paraphernalia.
To make matters worse, 40% of those infected don’t know they have it until symptoms progress, making it more likely to spread without proper intervention.
Luckily, when the infection is identified early, current treatments are over 95% effective. Treatments take only 8-12 weeks and cause very few side effects.
